To apply for financial aid please complete the appropriate forms:

You must complete the official "financial assistance" form and show proof of household income to be considered for financial assistance. Attach the past years Federal Tax Form and a final pay stub or W2. Any application, which does not include these forms, will not be considered for scholarship.
Financial assistance will potentially provide funds for REGISTRATION FEES AND OCSA FEES ONLY. Financial assistance does NOT include any team fees.
Application deadlines:
Fall Season - June 30
Spring Season - October 31
*Late applications will be considered only if funds are still available.

Information in the application will only be used in determining eligible candidate for dues assistance will remain confidential. All families who receive dues assistance are expected to attend all practices, games and team functions. All families who receive dues assistance will be expected to volunteer for a club event. Each family will be given a choice of events upon award of assistance.
